Signature 65 Transition to Freedom Blue PPO Medicare Advantage Plan
Effective January 1, 2025, all Medicare eligible retirees and dependents residing within the United States that are enrolled in Signature 65 with Medicare Part A and B (group numbers: 064222-00 to 064222-09) will transition to the new customized Freedom Blue PPO Medicare Advantage Plan.
