Effective January 1, 2025, all Medicare eligible retirees and dependents residing within the United States that are enrolled in Signature 65 with Medicare Part A and B (group numbers: 064222-00 to 064222-09) will transition to the new customized Freedom Blue PPO Medicare Advantage Plan. In the coming months, communications will be provided to retirees and dependents enrolled in the aforementioned coverage, as well as those becoming Medicare eligible later in 2024.
*NEW: If you recently received a letter from Department of Health and Human Services,
Centers for Medicare and Medicaid Services (CMS), asking you to complete a declaration
of prior prescription drug coverage, Highmark has already taken care of this with
CMS. There is no action you need to take. If you have questions or concerns about this, please contact Highmark at the phone
number provided on the form.
